What is P021, and why do suppliers call it P21?
P021 is a small peptidergic compound that Khalid Iqbal's research group described as a peptide mimetic derived from ciliary neurotrophic factor (CNTF). Kazim and colleagues, in a 2014 paper in Neurobiology of Disease, gave its sequence as Ac-DGGLAG-NH2, corresponding to a biologically active region of human CNTF (residues 148 to 151) to which an adamantylated glycine was added to increase stability and lipophilicity (source 1). Khatoon and colleagues gave the same DGGLAG sequence in 2015 and described the compound as blood-brain-barrier permeable (source 6). Baazaoui and Iqbal called it Peptide 021 (P021) in 2017 (source 2). Supplier pages commonly shorten the name to “P21” and do not all describe the compound the same way, so they were not used here. This Steadfast Research Group profile covers only the peer-reviewed literature, in which every study involved mice or rats and none reported human data.

What did the 2014 triple-transgenic mouse study report?
Kazim and colleagues gave female 3xTg-AD and wild-type mice a P021 diet or a vehicle diet for 12 months, starting at 9 to 10 months of age, which the abstract placed before tau pathology appears in this model. They reported a significant fall in abnormal hyperphosphorylation of tau at sites associated with Alzheimer neurofibrillary pathology, with no change in total tau. Soluble amyloid-beta was lower after 6 months but not after 12 months, and insoluble amyloid-beta 40 and 42 did not differ. The authors also reported rescue of deficits in neurogenesis, neuronal plasticity and cognition (source 1).
The authors proposed that the tau result was probably mediated by higher brain-derived neurotrophic factor (BDNF) expression and lower glycogen synthase kinase-3 beta (GSK3β) activity. That is a proposed route stated in the abstract, and the sources reviewed did not include a separate study isolating it (source 1).
What did lifelong exposure in 3xTg-AD mice show?
Baazaoui and Iqbal, in a 2017 paper in Alzheimer's Research & Therapy, added P021 to the diet of 3xTg-AD mice from 3 months of age, before any overt amyloid or tau pathology, and continued it to 21 months. Wild-type and transgenic mice on a vehicle-only diet served as controls. The authors reported that P021 rescued dendritic and synaptic deficits, increased neurogenesis and rescued cognitive impairment in the transgenic mice. They framed the result around neurotrophic support during what they called the period of synaptic compensation (source 2).
What happened when exposure began before birth or in early life?
Three further papers moved the exposure window earlier. In 2017, Kazim and colleagues gave P021 to the dams of Ts65Dn mice, a Down syndrome model, from embryonic day 8 to weaning at postnatal day 21. They reported that this rescued developmental delay in the pups and hippocampus-dependent memory impairment in adult life (source 5). In 2020, Wei and colleagues used the same embryonic day 8 to postnatal day 21 window in 3xTg-AD mice, followed by a standard diet, and reported rescued cognitive deficits at 4 months, reduced tau hyperphosphorylation and a lower amyloid-beta plaque load at 22 months (source 3). In 2021, Wei and colleagues started P021 at birth; as summarized in the 2026 imaging paper, it was given to postnatal day 120 and the authors reported rescued cognitive impairment and a lower synaptic protein deficit in 4-month-old 3xTg-AD mice, with higher pCREB and BDNF expression (sources 4 and 7).
What did the aged-rat and imaging studies add?
Khatoon and colleagues, in a 2015 paper in the Journal of Alzheimer's Disease, measured cerebrospinal fluid tau in aged Fisher rats. They reported that elevated tau in aged rats was lowered toward the levels seen in young adult rats after chronic P021 administration, and that amyloid-beta and AβPP did not show the same change (source 6).
The most recent paper used imaging. Falangola and colleagues, in a 2026 paper in Magnetic Resonance Imaging, used diffusion MRI in young 3xTg-AD mice given P021 or vehicle. They reported higher fractional anisotropy and axial diffusivity in the corpus callosum, and higher fractional anisotropy and kurtosis fractional anisotropy in the external capsule, compared with vehicle-fed transgenic mice. The authors described this as partial normalization of white matter diffusion measures, and they listed three limits of their own: no histological validation, no behavioral testing and a single timepoint. A corrigendum to the paper was later published (source 7), and its contents were not reviewed, so the corrected paper should be checked before any of its figures are cited.

How independent is the P021 evidence?
Khalid Iqbal is listed as an author on every paper in the table, so the findings come largely from one research network. The sources reviewed did not include a replication by an unrelated laboratory. The animal models are also narrow: five of the seven papers used the 3xTg-AD mouse, one used the Ts65Dn mouse and one used aged rats. The papers began exposure anywhere from before birth to 9 to 10 months of age, so they address when exposure started as well as whether it had an effect. The imaging paper adds a different readout, but its authors noted that it had no histology or behavior to anchor the diffusion measures (sources 1 to 7).
How do laboratories confirm P021 identity?
No analytical paper specific to P021 was among the sources reviewed, so the workflow below is the general one used for any research peptide. A mass spectrum is compared against the calculated mass for the stated sequence, and an identity result is paired with a purity figure from a separate method such as HPLC. Because the sequence includes a non-standard adamantylated residue, a batch record should state which structure was confirmed. The guides to reading a peptide COA, mass spectrometry verification and HPLC purity testing cover those checks.
What terms recur in the P021 literature?

- Fractional anisotropy
- A diffusion MRI measure of directional water movement used in the 2026 white matter analysis (source 7).
What remains unresolved in the P021 literature?
Several limits apply. The proposed BDNF and GSK3β route came from one abstract, and the sources reviewed contained no independent confirmation. The soluble amyloid-beta finding in source 1 appeared at 6 months and not at 12 months, so the amyloid results are mixed. Every study used rodents, so nothing in the sources reviewed speaks to human biology. The sources were read at abstract level, and administration amounts and many numerical results were not reported here because they could not be confirmed from the abstracts. Compounds with a neurotrophic framing are profiled elsewhere in the blog, including dihexa and Cerebrolysin, and each carries its own evidence limits.
Frequently asked questions
Is P21 the same as P021?
The peer-reviewed papers reviewed here use the name P021, also written Peptide 021, with the sequence Ac-DGGLAG-NH2. Supplier pages often write P21 and describe the compound inconsistently, so a supplier's description should be checked against the published sequence.
Which animal models were used in P021 studies?
The sources reviewed used 3xTg-AD mice, Ts65Dn mice and aged Fisher rats. No human study was among them.
Did the studies measure amyloid-beta as well as tau?
Yes. The 2014 paper reported lower soluble amyloid-beta after 6 months but not 12 months, with no change in insoluble amyloid-beta 40 and 42. The 2015 aged-rat paper reported no matching change in amyloid-beta or AβPP.
Has the 2026 diffusion MRI paper been corrected?
A corrigendum to the paper appeared in Magnetic Resonance Imaging in 2026. Its contents were not reviewed here, so the corrected version should be read before any figure from the paper is cited.
Why do the exposure windows differ between studies?
The authors tested different start points: before birth, from birth, from 3 months of age and from 9 to 10 months of age. The papers framed this as a question about when neurotrophic support was present in the model.
